Interconnect Health and Wellness is a primary care support service dedicated to helping you define, achieve and sustain your health. Through our holistic and nurse-led approach we support you in becoming more aware of your health and then navigating the next steps. Our care is personalized and client-centered, offering comprehensive nursing assessments, regular home check-ins, system navigation, or health education. We aim to empower and connect individuals and communities in their health journeys.

Our Services
All services are led by a skilled Registered Nurse and are designed to supplement, not replace, your current medical care. We advise clients to maintain contact with their primary care providers.
1. Initial Holistic Health Assessment
Every personalized care plan begins with a holistic nursing assessment. This is an in-home comprehensive assessment (up to 1.5 hours) that addresses physical, mental, spiritual and sociological areas of one's life. Included is a 30 min follow-up session to discuss the next steps. This assessment must be provided prior to receiving ongoing support and must be in-person.

2. Ongoing Support Options
Following the Initial Holistic Health Assessment, you may choose further support on a pay-per-service basis or packages for more routine care. Follow-ups can be in your home, or online.
Pay-Per-Service (starting at $100/hr)

Wellness Check-in (15-20 min): Brief updates on health status, vital sign checks, wellness tips, and encouragement etc.

Wellness Consultations (30-45 min): Discuss recent health changes, new diagnoses, or plan revisions etc.

Wellness Management and Care Coordination (60+ mins): Assistance with navigating multiple healthcare providers, managing complex care needs, and coordinating between providers etc.
